4/15/2023 EnchantaVaughn Approved Behavior: During the visit, Anthony displayed appropriate affect and easy-going behaviors as he engaged in an appropriate manner with his mom, sister, and brother in the home. Anthony was able to present on task behaviors such as Anthony practicing how to ask his mother to speak before just blurting out what he wants from her or QMHP. Behavior: Anthony verbalizes positive self-talk and affirmations around his family. Anthony expresses gratitude and compassion toward his counselor for helping him and being there for him. Anthony was polite and courteous to mental health professional while reviewing the psycho social activity. Anthony was applauded for listening, paying attention, and sitting in his seat correctly as Anthony was able to sit patiently and wait for the second portion of the session to begin. Anthony was praised by QMHPC for the positive behaviors he was displaying without needing redirection. Behavior: Anthony asked to lead the next activity and when doing so he spoke with confidence and high self-esteem. Anthony is observed displaying age appropriate manners, avoiding distracting behaviors. Intervention: QMHP provided individual and family counseling based on specific treatment objectives. QMHP initiated a rapport building conversation with Anthony in efforts to learn more of his strengths. QMHP monitored progress in demonstrating individualized pro-social skills (e.g. anger management, education, problem-solving, conflict resolution, appropriate verbalization of feelings, and cognitive-behavioral programming). QMHP recognized Anthony for his hard work, efforts, participation, and working towards improvement. QMHP complimented Anthony for utilizing acquired skills, paying attention, and focusing on the importance of in home services. Intervention: QMHP continued to establish a positive supportive relationship with Anthony. QMHP offered simple praises to the Anthony for behaving appropriately in the home and encouraged Anthony to practice the positive behaviors daily. QMHP supported Anthony displaying positive social interaction with siblings and parent. QMHP listened to Anthony verbalize his feelings towards making good decisions at home and avoiding conflicts. Positive feedback was provided to the Anthony as he displayed a clear understanding to how to use effective communication and assertiveness skills. QMHP encouraged Anthony to maintain his behavior and commended Anthony on his efforts to maintain his ISP objectives throughout the day. Intervention: QMHP led the family through each treatment exercise. QMHP engaged the Anthony in a role reversal exercise and required the Anthony to think about his goals for the future. QMHP utilized positive self-talk strategies to help Anthony think positively at all times. QMHP taught Anthony effective problem-solving skills (i.e. identifying the problem, brainstorm alternate solutions, select an option, implement a course of action, and evaluate). QMHP encouraged mother to allow Anthony to express himself in an age appropriate manner and to redirect when needed. Response: Anthony transitions smoothly and safely to session area, displaying appropriate behaviors. Anthony smiled and hugged QMHP. Anthony verbalizes positive feelings about himself and his family. Anthony interacted well within his home environment by implementing positive conduct and verbalizations. Anthony discussed mental illness and its causes as a family. Anthony displayed interest in learning about ADHD, as evidenced by him asking questions and maintaining a cheerful mood. Anthony reported a decreased stress level due to his modulation of social contact and being able to remain calm. Response: Anthony has become more involved in age appropriate social activities and was given positive reinforcement for these choices. Anthony did not display any outbursts during the role play exercise. Anthony learned to look for similarities rather than differences in others. Anthony was able to speak calmly and demonstrate his improved communication skills while engaging in self-reflection. Anthony demonstrated expectations well in the home. Anthony is making progress with expressing his feelings and showing others respect and courtesy by talking with appropriate tone and initiating healthy conversation. Response: Anthony was receptive and open to the interventions, as he was attentive and focused on the information and activities that were being processed without any difficulties. Anthony displays good social behavior, as observed and supported by mental health professional. Anthony and mom were observed interacting well with one another. Anthony is reported building healthy relationships outside of the home as well. Anthony utilized meditation and relaxation techniques. Anthony was seen decreasing anger and frustration. Anthony reported a decrease in symptoms following the relaxation techniques.
